Today I was honored to share my story for the documentary Suck It Up, Buttercup @suckitupbc_2025 with @otten_todd and Dr MaryAnn Wilbur.
Did you know that US hospitals tie nearly a third of their Medicare reimbursement to patient-satisfaction scores — surveys measuring how happy a patient felt, NOT their outcomes?
When physicians are graded on satisfaction rather than competence, we start ordering extra tests, writing extra prescriptions… doing whatever it takes to keep the score high — even when the science says stop.
Why?
It's why my TEDx Talk went viral here on X; It's Fear.
It's the fear of being written up, losing our jobs and reputations, merely for protecting medical integrity. That fear drives appeasement over ethics, and it increases the risk of real harm; the kind that betrays the very oath we physicians swore to Do No Harm.
It’s an impossible equation: protect your integrity, or protect your income.
And that’s how the metrics meant to measure care start bastardizing it.
We built a healthcare culture that rewards perception over clinical reality, where being liked matters more than being right. And that distortion doesn’t just hurt patients — it’s breaking the very people trained to heal.
Physicians don’t burn out because they stop caring. They burn out because they care deeply in a system that punishes integrity and rewards appeasement.

You won’t always feel ready.
You won’t always feel seen.
But show up anyway.
Discipline builds what doubt tries to destroy.
Keep going.

You deserve to be loved. You deserve to be respected. You deserve to be appreciated—just for being you. No title, no trophy, no achievement required. If no one’s told you today: You are worth it.
My @TEDx talk got a trigger warning because I dare to speak out against the kind lies harming patients and the sick-care system that punishes physicians to trying to Do No Harm.
Please take 11 minutes to listen to my rallying cry to put Health back into Healthcare. If you love it, please repost and share. If you hate it, tell me why because I will never stop fighting for your health. Amy Gutman MD, FACEP... https://t.co/YohOMQWdn9 via @YouTube
@NemaniSonia Dr. Sonia, thank you for your strength and grace. You earned your place, and your presence in medicine will make a lasting impact. We need more compassionate leaders like you in healthcare.
