What ist the aim of assessing pain in the Emergency Department? Providing analgesia to those patients who need it. Are multidimensional pain scales better than unidimensional pain scales to assess analgesic requirements in the ED? @replynickel#teamUSB

@tabakovic_senad Agree. Of course the base of the lung is also scanned for the search for hemothorax, and at the same time for basal sign of pneumothorax. The question is: does visualization of the lung point exclude tension pneumothorax?
@PhilippeLeConte Would do the same as @tabakovic_senad. Anamnesis (start, characteristics, and irradiation of the pain, risk factors) and clinic (peripheral pulses, jugular veins) could probably further help. So, despite mainstem EKG would guess for aortic dissection (aortic syndrome)
U.a. ich liebe mein Job!
@regine_sauter @NZZ Interessant! Facharzt Notfallmedizin jetzt! @NZZ @regine_sauter @sgnor_ssmus@UniSpitalBasel@replynickel @GDK_CDS
@tabakovic_senad Yes. The important differential diagnosis here is retinal detachment, but as you pointed out, the membrane would not cross the intersection of the optic nerve sheath. CAVE: no visualization of the nerve here, see great video examples by #emrap -> https://t.co/Fyzx3tMCvk
@tabakovic_senad At least by PSP size does not really matter: conservative management is mostly possible, as long as the patient is not symptomatic (i.e. dyspneic, and of course hemodinamic stable) -> https://t.co/dWx3dkewno progression after 3d has low probability, so US diagnosis should be ok.
@tabakovic_senad @nachkontrolle I think unstable patients which are transient responder benefit from CT scan, not sure if US in these patients really plays a role.
@tabakovic_senad @nachkontrolle I don't think so. (e)FAST remain a standard by unstable trauma patients, BUT we need new US protocols for stable trauma patients (i.a. use of CEUS).
@tabakovic_senad @PhilippeLeConte @jiconnoly@casualtysrus@NephroP No experience also. Diagnostic (and eventually therapeutic) corporal blood aspiration is fast and easy, but #pocus could eventually help management afterward, especially in the case of a fistula (https://t.co/sMCTYN9edH, https://t.co/wEen1CEhaB).
