The HSE has apologised for failings in the care afforded to a young Cork father, Anthony Hennessy who took his own life in a psychiatric hospital just hours after his admission.
However, evidence was heard that he was not deemed high risk enough for special observations. He was found dead in his room just a few hours later.
“On behalf of St Stephen’s Hospital, Glanmire, Cork, and its staff, I wish to express my sincere apologies to Anthony’s wife, children, parents and extended family for the failing in the care afforded to Anthony at this hospital on November 25, 2019,"
