Should we be recommending high intensity exercise in Parkinson's to prevent sarcopenia? Maybe. Sarcopenia is when muscle mass, strength, and performance are compromised with age and from being more sedentary. Kim, Byun and Jeon argue YES and have provided some preliminary data supporting this point of view in @ParkinsonismD.

My take: I agree with the authors that it is time we start talking about a sarcopenia in Parkinson's! We will need to generate more data on an optimal regimen for folks to implement at home. We need an optimal treatment strategy inclusive of diet and exercise and if high intensity exercise is eventually proven to be optimal, what options will there be for folks who can't participate in high intensity exercise? One big challenge is that randomized trials have revealed that 'dietary intake of protein prevents sarcopenia and muscle loss.' Adding protein is tricky in Parkinson's because in some cases it can affect the absorption of levodopa. Thus, we will need to thoughtfully think through a sarcopenia Parkinson's strategy.
