At a lot of programs, especially schools that may not have massive recruiting staffs or that cast a wider net nationally, questionnaires can be one of the first ways your name, film, measurables, academics, contact info, and interest level get into their recruiting database.
It does not guarantee you will be recruited, but it can help you get on the radar and make it easier for that staff to evaluate, organize, and track you.
Fill it out completely. Use accurate information. Attach your best film. Make sure your GPA, grad year, position, height/weight, phone number, email, and social media are correct.
