The Department of Health just repeated what is already the 2016 Regional Serious Adverse Incident Policy.

The Winterbourne View whistleblower got a job with the CQC. A very positive action.

The Muckamore Serious Adverse Incident report was completed in 2018 and it looked at Adult Safeguarding cases at the hospital from 2012-2017. Some extracts from the report ;

Prior to CCTV allegations coming to light in 2017, it would seem that Muckamore was the most inspected hospital in N.Ireland:

Priory to 2017 there was large number of serious concerns meetings with RQIA and Belfast Trust , anonymous concerns reported to RQIA &reports of crisis.

@BrigeneN @NI_SCC The Department of Health have said they don’t hold any information on what professional body , hospital care staff can register with.

The @NI_SCC were asked by the Inquiry if there was any guidance on the skills needed by social care staff to care for patients with complex needs , including autism and challenging behaviour.

@NI_SCC said they were not aware of any.

The two witnesses this morning are Bria Mongan and Ian Sutherland : Both were Directors in the Health Service.

@ForMuckamore @niahealth The Mental Health Commission reports after their inspections at Muckamore are now part of the evidence on the Public Inquiry website.

The Commissioners appeared to be very impressed by what they saw when they visited Muckamore.

All vulnerable Adult investigations at Muckamore had to be reported to the NI Mental Health Commission.

The NI Mental Health Commission last appeared at @niahealth in 2008.

In 2005 the North and West Belfast Trust carried out a review of policy and procedures to safeguard children and adults at Muckamore.

The report said "Muckamore is recognised as a centre of good practice in the field of learning disabilities".

It is unclear by whom.
