How to Interpret Anemia + Iron Profile Like a Pro
When Hb is low, don’t jump straight to Iron deficiency.
The iron profile + RBC indices tell the real story!!
✅ Step 1: Confirm anemia
Anemia = low hemoglobin (Hb)
WHO cutoffs (adults):
Men: <13 g/dL
Women: <12 g/dL
➡️ Next: look at MCV
✅ Step 2: Classify by MCV
🔹 Microcytic (<80 fL)
Think:
1. Iron deficiency anemia (IDA)
2. Thalassemia
3. Anemia of chronic disease (ACD)
4. Sideroblastic anemia
👉 Iron profile is MOST useful here
🔹 Normocytic (80–100 fL)
Think:
1. Acute blood loss
2. ACDmeans Anemia of chronic Disease
3. Hemolysis
4. CKD
👉 Iron studies help if chronic disease suspected
🔹 Macrocytic (>100 fL)
Think:
1. B12/folate deficiency
2. Liver disease
3. Alcohol
4. Hypothyroidism
👉 Iron profile less helpful here
✅ Step 3: Interpret Iron Profile

Serum iron
Ferritin
TIBC
Transferrin saturation (TSAT)
1️⃣ Iron Deficiency Anemia (classic pattern)
All the above Parameter will be low except TIBC
Ferritin is most specific...
💡 Key point:
Low ferritin = iron deficiency unless inflammation present.
2️⃣ Anemia of Chronic Disease (ACD)
All the above Parameters will be low except Ferritin which can be normal to high...
💡 Ferritin is high because it’s an acute phase reactant.
3️⃣ Thalassemia trait
All Parameters will be normal, iron and RBC may be increased..
💡 Microcytosis with normal iron -> think thalassemia.
4️⃣Sideroblastic anemia
All Parameters will be high except TIBC which may be normal to low...
