It's the "lucky cigarette" superstition. Smokers flip one upside down in a fresh pack and save it for last as a good-luck charm—hoping they'll survive long enough to smoke it. The tradition traces back to WWII soldiers (especially with Lucky Strike rations like in your photo), who did it for luck or to burn any identifying stamps first if they had to ditch a butt. These days it's mostly just a quirky habit.
