Radicular pain is fascinating - & unpredictable
20 years experience of this clinically - & then (unfortunately) my own experience has reinforced this
Me - textbook L5 distribution pain; i could almost trace the pain path with a pen - following the sciatic nerve, from posterolateral buttock, around the fibula head to the foot.
A pt the other day - far lateral L4 compression (these can be hugely painful adjacent to the DRG) - disabling lancinating pain, only to groin & thigh ๐คทโโ๏ธ
Understandably the focus being on the hip joint

This foot was asymptomatic, but showed signs of FHL tightness/weakness. These tests are a slightly crude measure of strength/function, but can be useful as a cluster to help understand the tissues in & around the big toe when teamed with a thorough subjective history.
Introducing a Novel Diagnostic Tool: The "broken wing sign" is a newly developed, non-weight-bearing physical examination test to detect gluteus medius tendon tears and muscle degeneration.
High Diagnostic Accuracy: The sign demonstrated high accuracy for detecting any gluteus medius tear, with an overall sensitivity of 81.8%, specificity of 80.0%, and a positive predictive value (PPV) of 91.8%.
Detects Severe Pathology: The test is particularly useful for identifying severe gluteus medius issues. An external rotation of โฅ30โ
(a "highly positive" sign) yielded 100% specificity and 100% PPV for a tear, and this degree of rotation strongly suggests a severe, likely massive tear requiring complex reconstruction.
