- 3-yr CNS relapse: 9.3% (no HD-MTX) v 8.1% (HD-MTX), adjusted HR 1.13 [0.82, 1.57]
- no diff in isolated CNS relapse: 5.9% v 5.7%
- no diff in relapse rates for any high-risk subtypes
- DHL/THL not included

Alice Di Rocco shares data from the first analysis of phase 2 FIL_RI-CHOP study evaluating ibrutinib + R-CHOP in untreated ABC-DLBCL with IPI ≥ 2 (N = 75).
ORR was 90% at cycle 4 and 85% at EOT. 2-year PFS was 77.7%. Grade 3/4 neutropenia and febrile neutropenia was 33% and 4%, respectively.

🔑 findings:
💡 CD19 CAR T-cell therapy leads to high rates of durable responses in patients with primary mediastinal large B-cell lymphoma (PMBCL): ORR 79% CRR 68% 2-year PFS 58.6%. Outcomes may be superior compared to other LBCL subtypes.
💡We observed significantly lower relapse rates in patients exposed to immune checkpoint inhibitors prior to CAR T-cell therapy with ~50% absolute reduction in relapse incidence in ICI-exposed (2-year cumulative incidence of relapse: 21.7%) compared to ICI-naive patients (41.6%).
💡Prior ICI exposure was not associated with higher rates or severity of early toxicities such as cytokine release syndrome and neurotoxicity.
❓More data needed to better characterize how ICIs impact CAR T-cell function and TME in patients with PMBCL

- t-AML (38%) & t-MDS (62%)
- Median OS of 4.5 months
- Risk Factors after matching : preLD MCV & higher ICANS grade remained significant
- CD28 domain trended towards higher t-MN risk
- Pre-existing Mutations: 85.7%, TP53 (50%)
