🗣Narrow-complex tachycardia (SVT) – Unstable? Immediate synchronized cardioversion is the priority.
➡️Unstable = hypotension, signs of shock, altered mental status (confusion), or ischemic chest pain.
📚Why? Extreme rates (>150 bpm) slash diastolic filling time ➡️ plummeting cardiac output, cerebral hypoperfusion, and myocardial oxygen supply-demand mismatch (ST depression is expected, not surprising).
➡️Per AHA ACLS (2025 algorithm):
Synchronized cardioversion first (sedate if feasible). Shock is timed to the R wave (“R-on-R”) to avoid the vulnerable T-wave period.
➡️In regular narrow-complex SVT, adenosine (6 mg rapid IV push + flush) can be considered if it won’t delay it’s ultra-short-acting, often diagnostic, and terminates many re-entrant circuits.
📚Why synchronization matters⁉️
🗣Unsynchronized shock risks the R-on-T phenomenon (PVC landing on the T wave during relative refractory period) ➡️ ventricular fibrillation. Regularity makes sync highly reliable and life-saving.
