As a rad onc, I’d be careful calling this causal.

The “radiation-related” category includes melanoma and prostate cancer, neither of which has a particularly convincing ionizing-radiation dose-response in BEIR VII or the atomic bomb survivor data. For melanoma, UV-A exposure through cockpit glazing seems at least as biologically plausible, and the invited commentary itself raises this for pilots.

The paper’s own numbers also make the claimed “specificity” more difficult to buy. Flight attendants had an OR of 1.21 (95% CI 1.14–1.28) for cancers classified as non-radiation-related. That is clearly elevated too, and looks more like a general cancer excess than a radiation-specific signature.

No dose-response was able to be evaluated by years of service or route either, which you'd want if this were really a cosmic radiation story. And no adjustment for reproductive history, circadian disruption (already IARC 2A for breast cancer on its own), or PSA screening intensity, all plausible drivers of exactly these cancer sites.

Very interesting occupational signal and absolutely worth pursuing with actual dosimetry. But “yes” is a stronger conclusion than a death-certificate study can earn.

@guiltedsociety @EricTopol I am a strong believer in ALARA(concept of minimizing radiation), but intermittent imaging studies and continuous multi year daily exposure should not be seen as one and the same. Over imaging is an issue but my personal opinion is that it’s over blown (re exposure not cost)

Maybe a hospital CEO that self-insures, can explain this to me.

For your medical benefits, for your hospital employees, why would you hire an ASO that requires a Pre Authorization, and then issues denials for the care that YOUR DOCTORS, the ones that work for you, recommend for your employees and their families ?

In other words, even though you are paying for the care, that your doctors recommend for your employees, the benefits administer that you hired , is saying. It to trust your doctors.

Do you not trust your doctors ? Or do you trust the ASO, probably owned by a big insurance company, to dictate care for your members, over your doctors ?

I just don’t understand why the CEO doesn’t just say, “trust our doctors”.

And one more question , do even know which life threatening episodes your employees or their family members have had denied ? Shouldn’t you want to know the severity and what it would take for them to get the care your docs require ?
