Definite neurosarcoidosis

Another presentation of neurosarcoidosis. This time, it looks similar to an abscess. Biopsy showed noncaseating granulomas. After 4 years of treatment, there is a significant improvement in the lesion appearance.

Parry Romberg syndrome
Slowly progressive facial hemiatrophy. It can be associated with brain changes as well. Etiology unknown but thought to be autoimmune. Patients can present with seizures and migraines.
Green arrows show asymmetry between both sides.

Steroid-responsive brain inflammatory process
Reminiscence of CLIPPERS. Atypical location, as here, the pons didn´t have any abnormal signal.
Brain biopsy was negative for demyelination and it showed perivascular&parenchymal lymphocytic infiltrates.
GFAP ab negative.

Fulminant Multiple Sclerosis (MS)

The patient started on high-dose steroids and plasmapheresis after week 8 but still progressed. Also received cyclophosphamide with no response either.

Pathology of the tissue demonstrated demyelination.
