The hospitals are so reliant on resident labor that they would collapse. (To the point where a true strike is questionably ethical- so most only partially strike)

This conversely makes such residencies far more stressful- I can't take a day off without burdening my coresidents.

These are frequently community shops where a residency program has been tacked on to a system that runs well with PAs/NPs.

But most academic shops don't have this model. Residents run the hospital.
