Recently our NT OHT came together to provide care services to tenants in some of the TSCH communities. Our team actively worked to address the care needs of each resident and suggested a care plan which included at least one of our 10 partners available to seniors! @cityoftoronto

Our new multi-organizational North Toronto OHT Neighbourhood Care Team (NCT) recently saved a life and ensured several others received the urgent care they needed. This demonstrates the collaborative model's impact for those in great need with a well-connected team.
Einat Danieli, Clinical Manager and Fran Wolfe, Nurse Practitioner at the NT Interprofessional Primary Care Team (IPCT) touring with UNISON Diabetes team in Toronto Senior Housing Buildings. They will be supporting tenants with diabetes at the building and help them stay healthy.
