“Canada approved a woman’s death four hours after a psych unit let her go”

Donna Duncan was 61. A February 2020 car accident left her with a concussion. Anxiety, paranoia, food aversion, and severe weight loss followed.

Her family doctor refused to support medical assistance in dying. He said her mental health needed treatment. She found two assessors who approved her anyway, calling her weight loss and frailty proof that death was “reasonably foreseeable.” She was not terminally ill.

Her daughters got a judge’s warrant under B.C.’s Mental Health Act. She was held after a suicide attempt, assessed, deemed capable, and released. On October 29, 2021, she received MAID at home in Abbotsford. Her family says they were told she was dead less than four hours later.

Police investigated and found no crime. Fraser Health withheld the assessment records. The privacy commissioner backed that refusal. Her daughters still cannot see the paperwork that cleared her.

This is the system Canada built. A woman in a documented crisis walked out of a psychiatric unit and into a death approved on the strength of her own starvation. Safeguards failed in plain sight.
