Thanks for the thoughtful question! Here’s how each one works.
Hydroxyapatite (HAp) is the main mineral that makes up enamel — about 97% of it. When added to toothpaste, the HAp particles physically deposit onto the tooth surface and fill in microscopic defects and demineralized areas. It’s a biomimetic approach: repairing the tooth with the same material it’s already made of.
Fluoride works through a chemical reaction. It converts hydroxyapatite into fluorapatite, which is significantly more resistant to acid. On top of that, fluoride inhibits an enzyme in cavity-causing bacteria called enolase, suppressing acid production at the source.
In short: HAp adds material, fluoride upgrades the material that’s already there.
Full disclosure — I personally do research on fluoride, so I might be a bit biased here. That said: since the two work through completely different mechanisms, a simple “which is better” comparison is tricky.
My personal recommendation leans toward fluoride, both because of the depth of evidence behind it and because of its multi-angle action — it doesn’t just strengthen enamel, it also suppresses the activity of S. mutans, the bacteria that cause cavities.
At the same time, hydroxyapatite is a great choice for people who can’t tolerate fluoride, or for young children where swallowing is a concern. The evidence base for HAp is growing too, so I see both as excellent approaches to cavity prevention.
