@ScaffoldingNews Sadly, towers like this are an accident waiting to happen. Always choose 'EN 1004' mobile access towers. They have in-built safety features that prevent them toppling or people falling.
The tower scaffolding wasn't suitable for the type of work being undertaken, wasn't erected by a person trained and competent to do so, was erected to a height above recommendation and was loaded with a weight greater than the safe working load stated by the manufacturer.
