💊Aspirina: la piedra angular. La aspirina reduce la oclusión de injertos y los eventos CV sin incrementar significativamente el sangrado postoperatorio.
✅Mantener aspirina perioperatorio.
✅Reiniciarla dentro de las primeras 24 horas tras cirugía (idealmente antes de las 6 horas).
✅Continuar de forma indefinida a dosis bajas (75-100 mg/día).
💊💊¿TAP Dual? En pacientes con alto riesgo isquémico y bajo riesgo hemorrágico, puede considerarse aspirina + ticagrelor o clopidogrel durante 12 meses (indicación mas fuerte en SICA que en SCC) para mejorar la permeabilidad de los puentes venosos (reducen MACE pero aumentan 🩸).
✅Reiniciar TAPD tan pronto como el riesgo de sangrado esté controlado.

Lp(a) vs CAC: Which One Wins?
Many clinicians assume that elevated Lp(a) makes CAC scoring less useful because Lp(a) is associated with non-calcified plaque.
This new study of 11,319 individuals suggests otherwise.
✅ Elevated Lp(a) increased risk across all CAC strata.
✅ CAC remained a powerful risk discriminator.
✅ Even with Lp(a) >50 mg/dL, individuals with CAC=0 had remarkably low absolute event rates over ~15 years.
The highest risk?
Lp(a) >50 mg/dL + CAC ≥300
(HR 6.12).
Perhaps the lesson is simple:
Lp(a) tells us who is biologically predisposed.
CAC tells us how much of that risk has actually become disease.
And those are not the same thing.

✔️La ICA sigue asociándose a alta mortalidad y rehospitalización. El tiempo al tratamiento importa: reconocer y tratar temprano mejora desenlaces.
✔️BNP <100 o NT-proBNP <300 prácticamente descartan ICA.🔎
✔️El tratamiento inicial debe enfocarse en: O² si SatO₂ <90%, diuréticos IV tempranos, vasodilatadores IV si PAS >110 mmHg, identificar causas reversibles (SICA, arritmias, infección, valvulopatía, TEP, etc.).🎯
✔️La respuesta diurética debe evaluarse en las primeras 2 horas: diuresis >300 mL, Na urinario >70-80 mmol/L, si no se logra → duplicar dosis y considerar terapia secuencial.💦
