Interesting I wonder if a different virus triggers Parkinson’s disease. The Spanish flu pandemic after the first World War did so, So flu vaccines may prevent Parkinson’s. More research needed.

Does DBS help for dystonic tremor? How about in POLR3A-related disorders? Yau and colleagues @ANA_journals have published a nice series followed by an important discussion. This group provides some data on thalamic vs. pallidal (GPi) DBS in this circumstance were the syndrome includes dystonic tremor.
Key Points:
- Biallelic POLR3A loss-of-function variants linked to hypomyelinating leukodystrophy.
- The authors show a series w/ a different specific splice variant c.1909+22G>A which was associated w/ adolescent-onset spastic ataxia without overt leukodystrophy.
- There were 8 cases.
My take: The two folks in this study w/ dystonic arm tremor provide interesting data on DBS outcomes in this somewhat complex scenario. We have learned through similar experience that if dystonic tremor is the most disabling feature we should at least consider starting with the VIM thalamic target, rather than the GPi pallidal target. The response to dystonic tremor is usually not as robust as in essential tremor, however it can be meaningful, and in many cases has a similar overall outcome. We need groups to publish their experiences, to better understand how to choose targets for DBS. It is interesting that in one of their cases, they later switched from GPi to VIM thalamus (DBS).
