National Ambulance Service Advanced Paramedics and Beaumont Hospital Physio & Occupational therapists, responding to low acuity 999/112 calls from older people
Pathfinder is a @slaintecare funded collaboration between @AmbulanceNAS&Beaumont Hospital OT&PT Departments. We aim to safely treat >65 year old 999/112 callers at home rather than ED.@DubFireBrigade, @BeaumontEDnurse,Primary Care, Geriatricians&MDT are integral to our new service
Congratulations to our colleagues in Letterkenny, forging ahead with initiative collaborative care pathways for older people within their homes with fellow community and hospital experts. Looking forward to hearing more & good luck with the write up!
As 2024 closes, so does our pilot on POC blood testing. What a year & what a success for ACP in NAS! Thank you to everyone who has supported us. We're delighted to have achieved so much-with added integration of local cardiac team to avoid ED admissions. Now, lets gets writing!🥳
@gracecphysio was delighted to attend the RAMI research awards, collecting a research medal on behalf of @PathfinderACP The thematic analysis of >450 patient satisfaction survey was a collaboration between Beaumont Hospital, @AmbulanceNAS & @RCSI_Physio

Huge congratulations to Catherine Kelly, Gary Gardiner and the entire Donegal Pathfinder team, winners of best poster in the "Access and Integration" category in today's #HSEConference24. Well deserved recognition of their innovative work in bedside blood testing.
@HSEchange_guide@baker_ross@frances_horgan, @RCSI_Physio consistent messaging from Mary and your stakeholder analysis on the development of the Pathfinder model. Learn as you do and facilitate enablers for future success
Many interesting insights from the SIG topic on Leadership for Integrated Care hosted by JP Harnett.
@baker_ross speaking on the opportunities to gain insights on what is required for Leadership for Integrated Care. What works for people, what are their challenges and obstacles. This research can ideally happen in parallel to creating integrated services-lets learn as we do.
Strong messages around the need to properly invest in Leadership for integration through communities of practice, mentoring/coaching and skills development.
Key message: there are new competences required for Leadership in Healthcare and we need to resource this sooner rather than later.
