This is the most vulnerable piece I've ever written, sharing the details of my traumatic fellowship experience.
I feared its consequences, but I have a story worth telling & I hope my vulnerability can help others and promote culture change for trainees.

Misleading headline-- not "one" population. Many populations of those with immunocompromise and/or disabilities. But a good read nonetheless, and a reminder of why so many of us continue to take this pandemic extremely seriously.

@Perkinslizm @drjosflynn@bui_anthony @jakemiller717 @jbcarmody@DrDarcyW@ASPNeph@SDBPeds@ab_peds Sooooo true!!! More medical care (barriers) dictated by insurers and state legislatures.... 😑 Of course we have cases that could use additional eyes, but that's not the norm. And many clear cut cases could (&should) be diagnosed by PCP. Artificial barriers.
@bui_anthony@ab_peds@aap BTW ABP said no b/c they didn't want to set a precedent for shorter subspecialty training. And I couldn't find a job that would/could hire me for this Gen Peds+DBP work b/c of who's allowed to make certain diagnoses; who insurers pay for certain things, etc, that req board cert.
@bui_anthony@ab_peds Bonus 3rd tweet. The @AAP subspecialty advocacy campaign last year was asking Congress to give more loan repayment for rural subspecialists. But I'm on PSLF path...my problem would have been solved w/ shorter training & ability to pursue my 1st plan-- Gen Peds w/extra DBP cert.
@drjosflynn@bui_anthony @jakemiller717 @jbcarmody@DrDarcyW@ASPNeph@SDBPeds@ab_peds At least for DBP I think the density argument doesn't hold up. Upwards of 15% of kids have developmental or behavioral problems that DBPs (or, even better, Gen Peds + 1 yr DBPs) can help address more efficiently closer to home.
@SamirShahMD@bui_anthony Unless maybe to increase pay for these subspecialties 🤔 the opportunity cost is pretty huge. DBP isn't even mentioned in this study but would probably be on par with or worse than Ado medicine.

@SamirShahMD@bui_anthony See my tweets above. This is the focus, but doesn't fit job market w/ other loan repayment options. I say this as someone w/ much higher than avg med school debt (~400k). It'd be nice but rural areas need Gen Peds +extra expertise. I doubt throwing $ at the problem is the answer.
