As someone who was offered the “option” of going home on antibiotics 4 years ago (I was 54) when I was in the ER, sicker than sick, with what a CT scan & lab work showed was appendicitis, I asked the doc what happens if I go home, appendix bursts, how will I know? He said “Well, if you feel a lot worse then you’d come back.” The doc said “Most European hospitals go with antibiotics.” I said “since I’m very ill now & live in the US, I am opting for surgery.” I had arrived after 24 hours of what I thought was a stomach bug, weak, wobbly & unable to stay awake. Surgery revealed my appendix had already burst probably before I arrived at ER & since I had a shunt drain in my abdominal cavity, I was shuffled off for a month in ICU because the mess from the infection could travel up the shunt & infect the spinal fluid. My blood work was seriously messed up and required further antibiotics after I got released. My situation was unique but the fact that a trip back home with antibiotics instead of an appendectomy would’ve probably killed me. Short story long: CT scan isn’t reliable for detecting a burst appendix so maybe err on the side of caution.

Three things missing from this conversation.
First, the group this helps most is the one the study did not enroll: the patient with equivocal CT findings. That is one of the hardest calls in the ED. Possible appendicitis but not definitive. Do you send them to an OR for a potentially unnecessary operation or send them home with a possible surgical abdomen? If antibiotics work for confirmed cases, the risk calculus for the equivocal patient just got dramatically better. Treat, observe closely, and you have bought yourself time without committing anyone to a surgery they may not need.
Second, look at how we frame the numbers. Surgeons see 44% eventually needing an appendectomy and call it a failure. Patients are going to see 56% avoiding surgery entirely and say sign me up. Same number. Completely different read depending on which side of the scalpel you are standing on.
Third, the protocol started with 3 days of IV antibiotics. Why? Brad Spellberg has compiled roughly 60 RCTs across more than 21,000 patients and 13 conditions, osteomyelitis, bacteremia, endocarditis, intra-abdominal infection, oral non-inferior to IV in essentially every one studied head to head. If oral holds for bloodstream and deep-seated infections, the case for a mandatory IV lead-in for uncomplicated appendicitis is not obvious. And an oral-forward pathway does something the current protocol cannot: it makes the non-surgical option available to patients who do not want to sit in a hospital for 3 days on an IV.

Kyron was last seen on the morning of June 4, 2010, at Skyline Elementary School in Portland, Oregon, where he was attending his school’s science fair. He has not been seen or heard from since. Kyron has brown hair, blue eyes, and wears glasses. He also has a distinct V-shaped strawberry birthmark on his forehead.
In 2025, the Multnomah County Sheriff’s Office finalized the process of digitizing the entire case. This work included scanning and documenting thousands of pages of reports, photos and evidence.
With the digitization now complete, the Multnomah County Sheriff’s Office is leveraging new technology tools to advance the investigation. This will allow detectives to process and analyze more than 5,300 written documents and more than half a terabyte of data associated with this investigation in a new and meaningful way.
To build upon and maximize this progress, the Multnomah County Sheriff’s Office has increased staffing on this case. In addition to the MCSO detectives assigned to the case, the sheriff’s office continues to collaborate closely with the Multnomah County District Attorney’s Office, the Federal Bureau of Investigation, the East County Major Crimes Team, and the Portland Police Bureau.

One of the most shocking things I witnessed today was this woman in pink screaming “you gon end up like Metcalf, you gone be pushing up daisies”.

I also witnessed several Karmelo supporters get extremely aggressive when ANY media tried speaking with them.
One female reporter went up to the group to ask questions (linked below) and they immediately got angry saying “we told y’all to stop coming the f*ck over here” (this was her first attempt at speaking to them).
At this point I started warning every reporter who was trying to speak with this group, as earlier, another local reporter had tapped on the shoulder of the man who leads their chants. This man immediately squared up and acted like the reporter had punched him.
I’ve been covering protests for 7 years and this is some of the most violent and racially charged rhetoric I’ve ever encountered.
This is my little sister.
She was born on Christmas Day, the youngest of six children. She had Down syndrome. Doctors said she would never amount to much and suggested that she could be aborted or sent away to live in an institution so my parents wouldn't be burdened. My parents disregarded that advice.
She endured multiple open-heart surgeries before she was three years old. She came through that and grew into an amazing human being. She was an accomplished speaker, advocate, and actor. She has an IMDB profile. She graduated with honors from high school and attended college classes. She learned to drive, got married, and lived a full, independent life. She was an inspiration to tens of thousands of people who attended her speeches.
She was an incredible daughter, sister, aunt, and wife, and a dear friend to countless people. She passed away recently and left a huge hole in so many people's lives. As she once said, "People who have Down syndrome can change the world. Being smart is not always what makes people happy. Loving and being loved is what makes people happy." She was so happy, and she brought so much happiness to others because she loved (and was loved) unconditionally.
I wish you had met her. You might not have murdered your baby.
