Every family is paying about $30k annually for healthcare they barely use, and that typically comes with a high deductible and out of pocket costs.
The median healthcare spend is $500 annually.
Now imagine an alternate version. Every American has a tax exempt HSA, but to participate, you must purchase at least a catastrophic insurance plan. If we allowed pre-ACA plans, we are looking at $6000-12000 per year. For the poor, instead of Medicaid or an ACA subsidy, the government just funds their HSA for them.
Now every family is putting $15k a year into an HSA. Most people would barely touch that. Maybe use some to pay for a DPC and the occasional lab or prescription. But by retirement, everyone would have $2-5 million saved up. That could pay for all lifetime medical expenses for 95% of Americans.
The only people who have to deal with insurance companies are those with catastrophic high cost events (trauma, cancer, etc). The only role government has is as a reinsurance to keep premiums low and help people with pre existing conditions get plans. Or for the truly destitute who burn through their HSA and cannot care for themselves (universal backstop coverage).
This would provide truly universal coverage, give money back to patients, simplify healthcare, and make every American richer.
A deeply personal story I’ve never shared publicly: Some years back I was thrilled to discover I was pregnant …with triplets! A multiple pregnancy automatically placed me into the high-risk category. I was referred to a high risk doctor and had many, many ultrasounds. Sometime between the 10th and 11th week I silently miscarried Baby C. The heartbreaking news, “no heartbeat” was given. Miscarriage was not new for us, but it NEVER gets easier with subsequent occurrences. I was terrified before each ultrasound that there’d be one less heartbeat. At the anatomy scan, my blood ran cold when they told me that one of the babies (I know this is small potatoes but they blurted out the genders and we had wanted to be surprised on their birthday) had very large verticals in his head. The possibilities about what that could mean were very scary. Every scan entailed more measurements with me holding my breath, praying they hadn’t grown larger. Paralyzing fear gripped me. Before they could ask (like they had done about selective reduction when I was carrying triplets) I said we were keeping BOTH boys. Period. Fast forward to the twins’ birth. W’s head and ventricle size were completely normal! On the very large size of normal but within range. Not only did he not need a shunt or have delays of any kind, but he is one of the smartest, sweetest, most athletic, most Godly and hilarious children you’ll ever meet. Modern medicine is wonderful and has its place. There is no doubt. Heck, I’m married to a physician! But I often wonder how many of us 70s & 80s (and prior!) babies could have withstood the scrutiny of constant, highly-detailed ultrasounds. Heck, maybe large in utero ventricles run on my husband’s side? We’ll never know. But one thing I do know for sure is I’ll never regret my faith in God and the 3 beautiful gifts, that are our children, he bestowed upon our family—-upon the world, actually.

He’s not representing his constituents nor able to get signed into law Trump’s policies—-which are enormously popular and demanded by the country. So, he needs to be demoted.
My question is what can be done? I’m an action-and-results oriented person. Those in medicine (or me, by proxy as I’ve been married to a physician 20+ years) know what’s going on but not the general public. Is there a grassroots effort to counter the strong lobbyists the massive hospital systems engage? If so, how do I help? If not, how can one be organized?
Talking about the state of the union in health care helps but that alone won’t bring about change—-and revolutionary change is urgently needed!

My dad, an outdoorsman & avid hunter who relied on traditional, wooden bird calls, would have been thoroughly impressed and he didn’t impress easily.

Your “nonprofit” hospital has an $8M CEO, a bond-rated real estate empire, and a tax exemption. But the facility fee on your 12-minute MRI is just to “keep the lights on for the community.” Sure.
Exactly. We’re dealing with the 25 year effects of CMS paying physicians less and hospitals more. The solution may be different for each specialty but for the surgical specialties the only option is to define the value of our services to the hospital and extract that back in the form of a professional services agreement. Most big systems don’t want to do that but hire a good attorney and make it happen!
Surgeon husband: From my calculations of observing him sit at the kitchen table after work, I’d say he spends an extra 15-30 hours a week on his computer on the work you’ve mentioned. Sometimes it’s almost another full work day and goes into the wee hours of the morning. It’s ridiculous.
Exactly! One of my ex-neighbors, an ex-con and non-American citizen from Columbia, owned several rural hospitals that he easily bought and ran into the ground. They served only as fronts to launder his drug money. We didn’t know him, but read about his story in the newspaper. He later fled the country. So scary he lived so close by!
But physicians are the ones feds look at suspiciously and forbid from owning hospitals. Make it make sense!🤦♀️
And while we are using car analogies, why aren’t car salesman (I call them loan salesman), accused of taking kickbacks when they get “bonuses” from auto financing lenders? Where’s the outrage? Why aren’t federal regulators pouncing?
We’ve been trying to buy our teen a pre-owned car for weeks. We’ve been all set to buy 3X. As soon as they hear we’ll be using our credit union for financing, suddenly the car—that’s been sitting on the lot for ages—has magically sold to another customer in the dealership. Sure.
