The newly launched NAPCHD Strategic Document of Diabetes Care can be seen as the gold standard for care home residents with diabetes. I hope that all care homes should begin to implement this guidance.

Our 3rd paper on metabolic frailty phenotyping in diabetes. Remember, frailty severity determines glycaemic goals but frailty phenotype should determine what glucose-lowering therapy to use.

Further work by Ahmed Abdelhafiz and I supports the idea that a different approach to glucose-lowering therapy is needed in managing varied frailty phenotypes in those with type 2 diabetes, and supports a precision medicine approach.

It is so sad and tragic that my dear friend Prof Tricia Dunning AM, from Geelong, Australia, has died this week. She was a great pioneer in the world of diabetes nursing!
I wonder how many clinicians can accurately assess the benefits of long term interventions (1.1.1) when planning treatment in various groups of patients in an objective sense?
Have just read the September 2021 NICE update on type 2 diabetes. Quite a responsive and well written document. Disappointed that frailty is mentioned only in a negative sense (1.6.9) for relaxing targets with no other real helpful mention of older people.
