I must admit that before reading this I felt like Freeman was some crazy guy perforating skulls with an ice pick as a pseudoscience procedure. But apparently it was not that extreme.

Though transorbital lobotomy could be done under anesthesia, the preferred method by Freeman was apparently to give between shocks of electroconvulsive therapy. After the second and third shocks for the first and second procedure (performed bilaterally).

Prior results were conflicting so Freeman tried to develop an easier technique and the neuro anatomy knowledge from the paper seems good.

Certainly the first cases were maybe not as encouraging as described and the tools developed were not sofisticated at all...

Interesting to read one of the original papers of Walter Freeman from 1949 describing technique and results of transorbital lobotomy.

Even though it sounds completely barbarian, reading it gives a lot of context.

Lobotomy was already being performed in prefrontal manner.
