Her course became particularly challenging when she developed massive bilateral pneumatoceles, leaving very little functional lung tissue
Yet over time, the pneumatoceles regressed and the previously compressed lung tissue recovered
Aubrey remained on ECMO for an extraordinary 233 days (5,592 hours) before she was successfully decannulated
After nearly 10 months in the hospital, she was discharged home on room air
Aubrey’s story
At just 4 months old, she developed severe RSV infection complicated by necrotizing pneumonia, PARDS & cardiogenic shock
She initially required VA-ECMO. As her cardiac function improved but her lungs remained severely injured, she was transitioned to VV-ECMO

بحث نُشر قبل أيام يتكلم عن فكرة غريبة فعلًا: Total Liquid Ventilation (TLV)، واستخدموا neonatal piglets لدراسة شيء أدق مما نتوقع 🧵👇

الدراسة توضح أن من التحديات الأساسية مو بس تحقيق gas exchange، بل إيجاد lung volume المناسب للمحافظة على airway stability بدون التأثير على hemodynamics
أحيانًا قيمة وحدة زي airway pressure ما تكفي، متابعة dynamic changes خلال دورة التنفس تكشف airway deformation ما درينا عنها

الـECMO يقوم بجزء كبير من gas exchange
فبدل ما يكون هدفنا:
“Normalize every ABG”
يصير الهدف:
Protect the lungs while they recover
يعني نستخدم
lung-rest / lung-protective ventilation
و��خلي الـECMO يتحمل جزء من عبء gas exchange
