This took 3 hours for me to get on paper. Three years ago? I’d spit this out in conversation without skipping a beat over cocktails….Now? This is your brain 🧠 …. This is your 🧠 after Pfizer and 3 years of saying you KNEW what was wrong but it fell on deaf ears (not all, just most)….So forgive any disorganization… I’m still trying to make logical connections. So consider it a cognitive exercise for me…

Being on the patient side these last three years I’ve had a different experience. I still have the knowledge of a seasoned NP but now I rely on the same system with scheduled appointments (instead of quick colleagues) for everything. A few thoughts…

Racemic drugs are pharmaceutical laziness to begin with.
For what insurance and drugs costs?
Clinicians and patients deserve a cleaner therapeutic window.

Enantiopure drugs should be 1st line when the original racemate loses exclusivity.
I get it. They deserve the profits from an incredibly expensive and lengthy process of approval. But, play by the rules and give a damn about the people that need the superior formulary.That’s why you made it right??? Wrong….

The clinical trials testing a new single-enantiomer often DO NOT INCLUDE comparisons against their older, generic, and racemic versions.
So when “their” RCTs do not include a head-to-head comparison , the enantiopure drug often does not demonstrate superior performance. If that’s not unfair enough? Then they use a process called “Evergreening”. This gives the same drug undeserved additional years of exclusivity for a change that was in most cases NOT NECESSARY.

This is a real barrier to medical providers and their patients. It is a deplorable corporate strategy where BIG PHARMA extends the patent life of a drug by making MINOR modifications for another monopoly. This keeps drug prices very high . I’ve not included the exception of formulary changes made for safety.

But hey, medical professionals have all the time in the world to fill out prior authorizations, in some cases a response after a week just for insurance to deny. ALL THIS to get the drug that is best for their patient….

Their chemo can wait or give a more harmful one (patient can’t tolerate it and causes delay in care or comorbidities)….Their blood thinner isn’t a priority so let’s cause a bleeding ulcer with aspirin (it still wasn’t enough and your patient stroked). Or, a safer pain medication changed to an unsafe short-acting crap with a higher risk of dependence (now you’ve created an addict)….

But hey…. Let’s all spend this time arguing about the construction of ballroom like it affects us more than this. Where is the outcry for the middle class being discarded in the health system? There wouldn’t be I guess…. Because the people that march and burn things lately have healthcare where everything costs zero (there are always exceptions).

What do I know from being an NP THEN and a regular patient NOW? The people that MUST have insurance benefits (personal conditions or a sick dependent) are enslaved to their coverage. But if that’s not enough, big Pharma will let horse die while needing THEIR medication then insurance companies will kick the dead horse while relentlessly denying any responsibility in the demise of said horse 🐴 . RANT OVER.

I was given my first Pfizer 5 years ago TODAY on 07/29/21. I was the only worker in my home and it was mandatory for the populations I treated. Health insurance, income, and the roof over our heads wasn’t something our household could lose. My degree and credentials took me nearly a decade of education and experience to achieve. JUNE 19, 2021. FAUCI KNEW he had a PE (blood clot in his right lung) just 5 months after Covid-Vaccination. He received the best treatment, was put on Eliquis (to thin blood and clots), and TOLD NO-ONE. This was 5 weeks before my husband and I got the vaccine for work in rural Alaska. He had every opportunity to prevent deaths and lifelong impairment…HE DID NOTHING. Take his license, take his money, and give him the punishment of the worst criminals. I wage you would need the world’s largest arena for the execution and it would be AT CAPACITY. I want a VIP seat so I can hear it when this disgrace of a human being DRAWS HIS LAST FCKN BREATH.
