At this time, numerous emergency crews are on the scene in Union Township, New Jersey, responding to and investigating a mass shooting at a busy restaurant. Reports indicate a barrage of gunfire erupted inside or near the crowded establishment. Officials have confirmed that at least six people were shot. One victim has been pronounced dead at the scene, while multiple others have been transported to nearby hospitals for treatment. Authorities have not yet released information on a suspect, possible motive, or whether anyone has been taken into custody. The situation remains active and is still developing.

A U.S. appeals court on Friday declared unconstitutional a nearly 158-year-old federal ban on home distilling, calling it an unnecessary and improper means for ​Congress to exercise its power to tax.

The 5th U.S. Circuit Court of ‌Appeals in New Orleans ruled in favor of the nonprofit Hobby Distillers Association and four of its 1,300 members.

They argued that people should be free to distill spirits at home, whether as ​a hobby or for personal consumption including, in one instance, to create ​an apple-pie-vodka recipe.

The ban was part of a law passed during ⁠Reconstruction in July 1868, in part to thwart liquor tax evasion, and subjected violators ​to up to five years in prison and a $10,000 fine.

Writing for a three-judge panel, ​Circuit Judge Edith Hollan Jones said the ban actually reduced tax revenue by preventing distilling in the first place, unlike laws that regulated the manufacture and labeling of distilled spirits on which ​the government could collect taxes.

She also said that under the government's logic, Congress could ​criminalize virtually any in-home activity that might escape notice from tax collectors, including remote work and ‌home-based ⁠businesses.

Is the Broselow tape reliable in children?

The short answer is no. We analyzed >200,000 kids and found that nearly 1 in 7 the Broselow under-estimated kids weights by >20%.

Of note, we are not the first authors to question the reliability of this tool. However, this is the largest analysis to date.

So what methods should we use? Previous systematic reviews have assessed this question.

The best option is to get an actual weight. Most ED's have resuscitation beds that can measure a weight, so if you have that, use it. However, that may not be feasible in the prehospital setting. The next best option is to ask the parents how much the kid weighs. Previous prospective studies have found this method reliable.

If neither of those methods are available, then use this study to be aware of risk of underdosing, particularly with weight-sensitive medications, such as sedatives, paralytics, etc.

Does a BVM with PEEP valve confer better preoxygenation? In this population of healthy volunteers, it performed better than a BVM without PEEP and a NRB across all of the metrics.

-53 volunteers, adults (normal, overweight, obese), and kids (6-12 years), which is great for broad applicability
-measured dependent lung ventilation via electrical impedance tomography
-primary outcome was fracture of expired oxygen
-15L/min of flow, PEEP at 8 for adults, 5 for kids

The BVM + PEEP group had a higher fraction of expired oxygen, better impedance measurements, and longer time to return to their baseline expired oxygen fraction.
