Apart from the issues of infrastructure and staffing capacity, this is pretty much the most difficult issue to address in Irish healthcare at the moment.
I’ve thought about this issue for many years, and there is no easy right answer.
Ireland spent ~€140m on anti-cancer drugs in 2012. In 2024 it was ~€600m. Despite this increase in spending, Ireland is just not keeping pace with international standards of care. And the pace of change in standards is accelerating, not slowing down.
If the reimbursement process does not use the extra staff from Mazars, and the new EU JCA to accelerate decision making, in my view the whole system will fall apart very obviously.
Kudos to @roinnslainte and @CarrollJennifer for taking this on. Any nudge in favour of cancer patients will help to stabilise the system.

On the topic of anti-cancer drug reimbursement in Ireland:
Until relatively recently, the role of a medical oncologist was to weigh up the probable benefits and toxicities of the best available anticancer therapy, and offer professional advice, based on data and experience on the best possible option. At the threat of being sued if they made a bad decision.
Increasingly, the best possible treatment option is not available to medical oncologists as an option.
The best possible treatment option is a question relating to published clinical trial data, international clinical guidelines, and professional opinion, for specific cancer patients.
All of the people currently charged with the responsibility of making reimbursement decisions for new cancer drugs in Ireland have a very difficult job.
As difficult as that job is, they do not have to sit in front of someone who has cancer now, requiring a clinical treatment decision.
Tomorrow, again, I will politely ask a drug company for free drug supply.
The balance is not right.

In a searing interview with the BBC, paediatrician Dr Tanya Haj Hassan condemned the broadcaster for echoing Israeli government talking points as she recounted her harrowing experiences in Gaza.
Dr Haj Hassan, who recently returned from volunteering with Medical Aid for Palestinians, shared the devastating story of her colleague, Dr Alaa, a paediatric emergency doctor at Nasser Hospital.
While Dr Alaa was on call at the hospital, her home was bombed and nine of her children were killed, their bodies charred and dismembered beyond recognition. Her husband remains in critical condition.
Dr Haj Hassan described a healthcare system in collapse: intensive care units overflowing with maimed, orphaned children – many the sole survivors of their families.
She also questioned why the BBC has delayed the release of 'Medics Under Fire', a documentary it commissioned about systematic attacks on Gaza’s healthcare workers, calling for its immediate release.
