Small changes, big impacts in the classroom part 1: Test Talk.
For the first 5 minutes of a test, students put their writing utensils on the ground, I hand out the test, and students talk to each other about the test. Many have mentioned how much this lowers their test anxiety.

The discussion of math anxiety is proof that we don't understand/appreciate it's precursor--alienation. Students don't care about math if the subject has not been made attractive to them. Anxiety is really about testing. Alienation is about not liking the subject to begin with.
