I have never pretended anything different. There is a clear agenda to deskill doctors and substitute them.
It is a shame that there are nurses out there who are happy to facilitate the displacement and substitution of consultant doctors supervising registrars - the consultants of tomorrow.
That is not teamwork.
That is not support.
That is not the MDT.

On the train today, I was talking to a family member about going to see their GP.
A man overheard and cut in: “GPs are useless. They don’t do anything. You’re better off seeing a specialist.”
The reality for GPs? Ten minute appointments to unpick complex problems. Hundreds of clinical decisions a day. Hours of paperwork after clinic. Years of training to cover multiple branches of medicine.
All dismissed in seconds.
I told him I’m a GP trainee. I said GPs are working under impossible conditions, many burning out or leaving. His response? “No great loss then.”
It’s easier to blame the doctor in front of you than the politicians that created these conditions. But without doctors, the NHS cannot survive.
If we keep losing doctors, the NHS loses its foundation. That’s why we’re fighting for fair pay and working conditions: because without the staff, there is no NHS.

I’m backing Malinga, Cheska and Elliott for RCGP council. I’ve worked with all three; they fight hard for GPs and know what matters on the ground.
If you want a stronger RCGP, vote. We can’t afford complacency.
The GMC needs a hard reset:
🔹Clear out current “leadership”.
🔸Full democratisation of the council and all key leadership positions.
🔹Binding vote of no confidence processes.
🔸Return to doctor only regulator status.
🔹Spending surplus rebate mechanism for working doctors.
21% of GP networks in England have reduced their use of PAs in the past year.
Key reasons include:
- Patient safety concerns and rising complaints
- The supervision burden placed on GPs
- Guidance from professional bodies
General practice is a high risk environment with undifferentiated patients. It requires close to a decade of training to qualify as a GP, yet a physician assistant can enter the field after a two year postgraduate course.
At the same time, many qualified GPs remain unemployed. It is difficult to justify hiring PAs over GPs trained specifically for this work.
This is not the fault of individual PAs, but of a system that has placed them inappropriately and exploited them.
The priority must be to hire GPs to work in General Practice, as they have been trained to do @wesstreeting.

UMAPS has LOST its application for an interim injunction, pending the hearing of its JR claim, to stop NHSE / SoS from implementing the Leng Recommendations.
UMAPS wanted an interim injunction to preserve the current status quo (& uncertainties) pending the resolution of its JR claim.
The High Court dismissed the application.
Nash & Prof Leng were in court.
UMAPS intends to add Prof Leng as the third Defendant to the JR claim.
The fact that the Court dismissed the interim injunction application does not mean UMAPS will fail in its JR claim.
What an unbelievably arrogant way to handle this.
Doubling down on promotion of the CV of one candidate when simultaneously trying to apologise for promoting them.
This is a farce.
I don’t know this doctor but they’re standing for election in the college.
The college should do better and respect a period of purdah rather than giving an obvious promotion using college resources.
This is unfair and unreasonable.
The college needs to make amends and reach out to all nominees to provide them with similar level of exposure.

My mum today was talking about how she can’t get a GP appointment and has given up.
She was shocked when I told her there are thousands of doctors who want to be GPs but the government won’t fund the healthcare system properly, so they were turned away from GP training.
When you book a GP appointment, you expect to see a GP. But that’s becoming a luxury.
More and more patients are being seen by anyone but a doctor. Meanwhile, qualified GPs are out of work because there aren’t enough jobs for them.
That’s not by accident. It’s by design.
Yes, there’s a role for other valued healthcare professionals in general practice (with a set scope of practice and for specific tasks) - but not as replacements for GPs.
We risk sleepwalking into a two tier NHS: where only those who can pay get to see a doctor, and everyone else is left behind.
What is happening in the NHS?!
Now there is NO REQUIREMENT to have a SINGLE DOCTOR ON SITE in Urgent Treatment Centres?!
