It is great that so many people are concerned about Food Safety and cleanliness in the kitchen. But we need to clarify something about the use of gloves. Often when I share videos of cooking viewers will complain that the chef is not wearing gloves. Gloves are not always necessary and sometimes they can be worse than bare hands. If you are handling food that is to be served raw then gloves should be worn. But if the food is cooked, heat kills all pathogens so gloves probably aren’t necessary. If you are handling food that has been cooked gloves should probably be worn. But a lot of cooks forget to change the gloves. If you’ve handled raw chicken with gloves, you need to change the gloves before you handle cooked food or raw food. In many cases, frequent handwashing is better than wearing gloves. That said, something that is often overlooked is that every handwashing station should have a fingernail brush and cooks should keep their fingernails trimmed short and that applies to all genders.

15 health care trends

1. Supply and demand remains the top challenge in health care. Approximately 900000 practicing physicians for a population of nearly 350 million and growing.

2. With supply and demand becoming increasingly out of whack we suffer in all three categories of the triple aim. Access is more and more challenging, costs are up to more than $30000 a year for a family of four and the spectrum on quality is very broad -‘s hard to broadly say quality is improving when we have harder and harder challenges getting the right care at the right time.

3. The depth of specialists and subspecialties is getting thinner and thinner. Even at the very best institutions, we increasingly find that the bench is very short.

4. Average costs for health care have grown to approximately $34000 a year while median income is right around $85000 a year.

5. Value based care, capitation and fee for services each have distinct challenges. We can fiddle with the payment system as much as we want but without solving supply and demand we can’t truly improve care and make progress on the triple aim.

6. AI is cascading into having many different use cases, nurse education in our country has accelerated tremendously in our country over the last four to five years and we are seeing great movement such as in GLP prescription and management away from the traditional drs office and health systems.

7. The payors are massive. The four largest payors are four of the largest 20 companies in the United States by revenues. While they may complain about medical loss ratios moving back up to 87 to 89%, this means at that level we are still spending hundreds of billions on administration of health care versus providing care.

8. Washington DC should focus a lot more on how do we provide health care for everyone in a high quality way and less on political platitudes. You can talk all you want about coverage for all and most of us want that but adding that without solving supply is just politics.

9. The evolution of concierge care is a direct result of supply shortages. More and more need to pay an access or concierge fee.

10. We provide massive subsidies through the insurance companies and this leads to increased health care inflation. Then we have no way to stop those subsidies because people can’t afford insurance without the subsidies. It’s the same concept with our national debt and how it leads to inflation. We need to slowly wean ourselves off of some of the huge subsidies.

11. Value based care is a managed care made up term.

12. Medicare Advantage costs more than $500 billion a year and thus far has been more costly than traditional Medicare.

13. The best health care leaders do a tremendous job of sorting through the noise to focus on a few key priorities.

14.More primary care will go through PA and NPs. More anesthesia will go through CRNAs.

15. Medical education needs to be dramatically changed.
