For those like me heading into work for a day of MSK triage, remember this. Most of what you will see is non specific, variable, and hard to slap a diagnostic label on. Your scan results will often be equivocal and you will not be able to provide certainty to your patients based purely on a pathoanatomical model. And you know what? That is ok.

It is real life, real people, real complexity. Not everything is knowable and your patients are not from a textbook. If it does not fit, do not try to make it fit. Be honest, communicate, and manage expectations. Do not feel you must always meet or exceed them.
You are human too and we do not have this stuff figured out. Not by a long shot.

Today’s clock rules:

1. The iPhone clock automatically went back 1 hour.
2. Wall clocks to be wound back 1 hour.
3. It will take 1 hour to work out how to reset the oven clock.
4. The car clock will remain 1 hour ahead of time until the clocks go forward in March.

What I consider to be 'AdvAnCEd' MSK practice (spoiler: do the basics well).

✳️ Listen more, talk less (at the beginning).

✳️ Use a basic exam to educate.

✳️ Offer a solution that requires as little change as possible.

✳️ Think of the end at the beginning.

✳️ Know when a scan or test is superfluous to management.

✳️ Manage expectations; don't always try to meet them.

✳️ Know some non-MSK differentials, but also know when it's better for someone else to take a look.

✳️ Don't order an investigation that you personally won't act upon.

✳️ Meet people where they are. Big things have small beginnings.

✳️ Chat about the weather or whatever common mundanity the day brings.

✳️ Be honest about prognosis.

✳️ Know your comms will sometimes be crappy.

✳️ Understand that some people will be rude, and it's not about you.

✳️ If the story doesn't make sense, keep asking questions until it does.
