Yes, the causality analysis of composite variable outcomes (or exposures) such as BMI or weight change is subject to inferential bias! Composite variable bias; just published in IJO:

@biostatsfun@IntJObesity As we have mentioned in our paper - using composite variables as either exposure or outcome (except when serving as proxies for latent variables) carries the risk of producing misleading causal effect estimates.
Even in the wake of a devastating earthquake that has killed 5,000 people in Turkey and Syria, the US and EU refuse to lift the sanctions which prevent Syrians from receiving direct aid from many countries.
