Pronase IF critical in Light Chain Proximal Tubulopathy. Routine IF completely negative, but pronase IF shows kappa light chains with crystalline shapes. LM showed ATN. I would have missed the LCPT diagnosis if I did not perform pronase IF. 55-yr woman with CKD and abnormal FLC.
Acute tubular necrosis (ATN) can persist months in the setting of severe BURNS. Interstitial edema (A,B. bluish material between tubules, thin arrows) and Rhabdomyolysis (C.Necrotic debris in tubules and D) myoglobin stain) are big culprits. 40-yr man with ARF months after burns.
