-Pts with CD on combo therapy + steroid free remission > 6 mo ~similar relapse rates between IMM withdrawal versus continuing combo therapy (10% vs 14%).
-A/w higher risk complications s/p endo dilation = multiple strictures in short segment, deep ulcers, stricture > 4-5 cm
Ex: macular edema
reversible but risk higher in DM. Consider relative contraindications based on patient hx.
-CRP & WBC lvl can be predictive of relapse risk at Year 1 & Year 2 of weaning off IMM in combination therapy.
Good reviews and discussion this morning:
-TNFi dose reduction more successful if no prior TNFi failure + deep remission.
-S1P receptor mods AEs = guided based on patient-specific factors, positioning after 5-ASAs.
