@nickmmark Not sure why potassium would ever be batched in syringes. We dilute to at least 0.4 meq/mL, usually 0.2 meq/mL and infuse at 10 meq/hr. We use barcode scanning and visual inspection for all compounds, trace NDC, lot, exp for each component.
@nickmmark Its possible they were making multiple batches at the same time and swapped labels. Bad practice, but I've seen it happen and lead to errors. Zooming out, not sure why syringes of potassium would be needed? Always run slow 10 meq/hr diluted to at least 0.4 meq/mL
@nickmmark Epic then generates a label that can be used to trace history of compound. Also wont allow scanning for admin if BUD has passed. 100% compliance expected. We do catch things from time to time, nothing as egregious as potassium being used in place of bupivacaine
@kevinnbass Interesting question. Lots of variables, so hard to make a generalization. In my experience monitoring therapeutic levels of drugs like vancomycin and aminoglycosides (no metabolism and fully reliant on renal function for clearance), I would say no, at least not significantly
