They happen because of poor leadership and usually not seeing the future.

ESPN completely whiffed the streaming race, and their revenue stream is paying for it.

They should have grown the rough and narrowed the fairways first. Heck, even raise the fairway height! Change the architecture to make it penal to hit it 350. Make them hit it 270-300.
