Zitzmann et al. highlight that deciding whether to save or replace a questionable tooth requires integrating periodontal, endodontic, implant, and prosthodontic factors rather than defaulting to extraction and implants.

2️⃣ Periodontal perspective
Residual probing depths, attachment loss, and furcation involvement are major predictors of tooth loss, yet periodontally compromised teeth can remain functional after successful therapy and maintenance.

3️⃣ Endodontic perspective
Most failures of endodontically treated teeth are due to prosthetic or periodontal issues—not endodontic causes—and crowned endodontically treated teeth show significantly higher long-term survival.

4️⃣ Implant perspective
Dental implants demonstrate high survival rates, but failures related to peri-implantitis or overload often require implant removal, with certain patient factors increasing risk.

5️⃣ Prosthodontic perspective
The amount of remaining coronal tooth structure and strategic value of the tooth within the overall plan are critical determinants of restorability and long-term success.

6️⃣ Clinical takeaway
Implants are not inherently superior to natural teeth; evidence supports a conservative, case-specific approach that balances biology, prosthetic design, and patient factors.

Root Canals & Vertical Root Fractures In Patients With Type 2 Diabetes

Type 2 diabetes mellitus patients was 2.6x increase with VRFs in crowned, root canal treated teeth vs normal patients.

Root dentin from patients with type 2 diabetes mellitus and VRF had higher levels of dentinal sclerosis, pentosidine, collagen cross-linking ratio and mineral to collagen ratio.
