@DrPJVDK@generalorthomd@stevemchale I’ve only had 2 dislocations in over 700 Oxfords, both were solved with poly exchange and removal of impinging bone. Loosening is by far my biggest issue leading to revision. I also have been eagerly awaiting approval of the press fit Oxford for over 10 years now.
@generalorthomd@stevemchale That’s surprising, poly instability is very unusual in Oxfords from my experience. I assume they were having a lot of medial pain?
@northwoods1980 Agreed, these are challenging cases. My feeling is at least correcting the cam deformity and saving the labrum if possible is helpful but isn’t always enough. I’m unsure if more than that is the answer, as the next steps start to involve fairly morbid procedures. Tough.

The NCAA needs to allow Bellarmine full access and rights in Division I, and do so immediately. Denying it is discriminatory and flat-out wrong. So much for “athlete welfare.”
@jointdocShields As a sports surgeon, I am referred so many of these (native and post-arthroplasty) and many are very symptomatic, however some are so chronic that repair can’t always fix the limp but pain typically is improved. I don’t do THA but I would assume there is a significant % >70 yo
@FixnBones I see so many of these now, chronic bursitis that doesn’t respond to injections really raises my suspicion for these tears. I do them open as well, even though I do a lot of hip arthroscopy. Unfortunately I also see many chronic tears with muscle atrophy that don’t do well.
@DrDerekOchiai What are your thoughts on the psoas tendon itself causing the impingement in these tears? I used to release them more often but now almost never.
@DrDerekOchiai I agree that placing from the articular side is key for these, I now use a 1.4mm all suture anchor and can often use a straight drill guide but curved is available.
