The CVP Waveform
a → c → x → v → y
a = atrial contraction
c = tricuspid valve bulging into the RA (RV contracts)
x = RV contracts downward (pressure drop)
v = preload filling the RA (early preload)
y = tricuspid opens → early RV diastole
This Memorial Day, we pause to remember the brave men and women who gave their lives in service to our nation.
Let us never forget their courage, their sacrifice, and their legacy.

A recent RCT of intubated patients with septic shock requiring pressors found that a ketamine bolus (1 mg/kg) provided improved hemodynamics compared to a fentanyl bolus (1 mcg/kg).
-86 participants randomized
-Must have been intubated with septic shock requiring pressors
-Randomized to ketamine or fentanyl
-Intensivist doing the measurements blinded to intervention
-Ketamine compared to fentanyl resulted in increases in heart rate, stroke volume, cardiac output, and MAP

Are you giving blood prehospital? 🩸 If so what?
If you don’t have blood, how are you treating hemorrhagic shock?
How much fluid are you giving and what blood pressure are you targeting?
Today, we honor the bravery and sacrifices of our veterans who have served and protected our nation. Your courage, dedication, and commitment to preserving our freedom and security are deeply appreciated. Thank you for your service.

I love this method for RSI we use on flight: the 3 stop-cock IV method.
Closest to patient: sedative
Middle: paralytic
Farthest: push-dose pressor
