AIIMS Guwahati Expands Inpatient Capacity with Inauguration of 120 Additional Beds and Senior Citizen Registration Counter
A Historic Milestone for AIIMS Guwahati!
AIIMS Guwahati achieved another significant milestone in its commitment to strengthening healthcare services with the inauguration of 120 additional inpatient (IPD) beds, increasing the institute's total IPD bed strength to 561. The occasion also marked the inauguration of a Senior Citizen Registration Counter, aimed at providing a more convenient and patient-friendly registration process for elderly patients.
The facilities were inaugurated by Prof. (Col.) Ashok Puranik, Executive Director, AIIMS Guwahati, in the presence of Prof. Vineet Mahajan - Medical Superintendent, Deans, faculty members, nursing officers, and other healthcare staff. The occasion was also graced by Mrs Manjusha Puranik.
The addition of 120 IPD beds will enhance the institute's capacity to cater to the increasing demand for specialized inpatient care, while the dedicated Senior Citizen Registration Counter reflects AIIMS Guwahati's commitment to ensuring accessible, efficient, and compassionate services for the elderly. Executive Director acknowledged that this achievement is a testament to the unwavering commitment and teamwork of the entire AIIMS Guwahati fraternity in strengthening patient care and institutional excellence.
The gathering also conveyed warm birthday wishes to the Executive Director and expressed gratitude for his visionary leadership and unwavering guidance, under which AIIMS Guwahati continues to expand.

AIIMS Guwahati Achieves Success in Complex Aorto-Ostial Left Main Stenting Procedures under IVUS Guidance
The Department of Cardiology, AIIMS Guwahatisuccessfully performed two complex aorto-ostial Left Main (LM) stenting procedures in unprotected left dominant systems under IVUS guidance with temporary pacemaker backup support. Both patients had an uneventful recovery and were discharged successfully.
These highly challenging interventions reflect the growing advanced cardiac care capabilities at the institute under the visionary leadership, constant encouragement, and guidance of Executive Director Prof. (Col.) Ashok Puranik, whose continued support has been instrumental in strengthening super-specialty services and promoting advanced interventional procedures at AIIMS Guwahati.
The Departments of Anaesthesia and Medicine provided invaluable collaboration and support in the successful management of these cases. Special thanks to Meril Life Sciences for their support with IVUS guidance.

I would say that the KISS trial @JACCJournals not only proved that you do not need to do routine SB intervention (commonly kissing), rather I would say the most significant change is proving the concept of POT-side-POT (done in 57% of SB interventions and seems to be more favorable than kissing).

I have been recently doing POT-side-POT instead of kissing when I need SBI and that study proved it can be better!

An important point for #MedTwitter:
Iron deficiency is common in patients with Eisenmenger syndrome despite markedly elevated Hb levels.
Secondary erythrocytosis from chronic hypoxemia increases iron requirements, impaired absorption, chronic blood loss, and prior phlebotomy —> worsen deficiency.
In cyanotic CHD, iron status should be assessed using ferritin and transferrin saturation , not Hb levels.
If iron deficiency is present, these patients often require oral iron supplementation, and at times IV iron as well.
Too often, iron is stopped by physicians or pharmacists,because the Hb is “too high.”
Secondary erythrocytosis does NOT mean adequate iron stores.
Iron deficiency can worsen hyperviscosity symptoms and may increase thrombotic/cerebrovascular risk in Eisenmenger syndrome.

Coronary fat embolization for late coronary perforation.

I did a routine DK-crush bifurcation PCI. 6 hours later, pt is hypotensive with a large effusion. Relook angio showed diagonal tip perforation that wasnt there at the end of PCI. 
Prolonged balloon tamponade failed. Coils unavailable.

Treated with autologous fat embolization, an effective bailout technique.
• Small 0.5 cm groin incision
• Subcutaneous fat harvested using forceps into small bowl
• Fat mixed with saline
• Loaded into a 2 cc syringe
• Using a microcatheter (Turnpike/Finecross/Export, etc.), to inject fat into the perf site.
• Final angiography confirmed complete seal.

Nature finds a way , the epicardial collaterals have formed just around the stented segment just like a moneyplant grows over a thread trail ..

Important question

Should we even open this CTO , which in all possibilities have 25% chances of reocclusion over less than 10 years , or just give medical therapy a chance now since we know these bridging epicardial collaterals are resistant to atherosclerosis and will never close down antegrade flow ..
