Trans women athletes are required to be on HRT as a prerequisite to compete in women's sports.
Whilst testosterone deprivation cannot shrink height, suppressed testosterone does result in reduced bone mineral density and reduced muscle and inhibits the ability to build as much muscle as those who have their normal or high concentrations of testosterone.
Testosterone suppression also reduces hemoglobin concentrations to cisgender female levels in just 3 months, while estrogen supplementation increases the total body fat
% in trans women by between 20 and 34% in just the first 12 months.
This results in what we call reduced fat-free mass. Fat provides resistance for movement, as opposed to a muscle, which provides torque for movement. Consequently, the more fat-free mass someone has, the better athlete they are likely to be independent of sex or skill level.
Couple this with the reduction of the oxygen-carrying hemoglobin in blood to cisgender female levels after around three months of GAHT, and a trans woman's body will be significantly underpowered. Trans women at 3 months of T deprivation have less delivery of oxygen to their working muscles produces a higher rate of metabolites and consequently results in muscle fatigue.
Based on this data, a cisgender male and a trans woman cannot be equated.
Trans women are physiologically different from men even before the onset of GAHT, hence I believe the basis of any blanket ban policy is flawed.
In summary
There is very low participation of trans people in sport due to a large number of barriers such as actual or anticipated discrimination.
Hormone therapy leads to substantial physical changes in trans women, which often narrow or eliminate many performance differences over time.
Any remaining differences are small and vary by sport. Some studies show a slight residual advantage in certain measures (e.g., handgrip strength) and a disadvantage in others (e.g., lung function and aerobic fitness). How much this matters depends on the demands of the specific sport.
Safety concerns are mostly hypothetical. To date, there is no solid evidence that trans women pose a greater injury risk to cis women in sport.
Fairness and inclusion can coexist. With thoughtful, evidence-based policies, both can be achieved without resorting to blanket bans.
