@GilbySusan @Maureenprsb I was very impressed that my local team sent a status email (alert level of each hospital) which included community status (district nurses, community beds) and now includes primary care status too. Nothing about who’s working harder than who - just “this is where we are all at”.

Early (vs late) physical therapy assessment for older people admitted to hospital associated with 29% higher probability of discharge to usual residence within first 21 days after admission. More reason to support early proactive MDT intervention to promote successful discharge.
