The controversial JAL crash in India – Flight 471 accident was 54 years ago
June 14, 1972- Japan Air Lines Flight 471, a DC-8, crashes in Delhi (India). 82 of 87 aboard and 4 on the ground die.
On approach to New Delhi airport, at night, the aircraft impacted the ground on a CFIT – Controlled Flight into Terrain – in an urban area 13 miles from the runway.
Indian investigators blamed the pilots, pointing to “total disregard of laid-down procedures by the crew and abandoning all instrument indications without properly ensuring sighting of the runway.”
On the other hand, Japanese staff assessing the enquiry stated “a false glide path signal to be responsible for the premature descent into terrain”.

This little-known occurrence in rural US became a watershed event for aviation safety
March 30 1979: Downeast Airlines Flight 46, a DHC-6 Twin Otter, crashed in Maine (US). Of 17 aboard, 1 survivor. On approach to Knox Airport (Rockland), the aircraft went under minimum safety altitude and impacted trees nearby in a CFIT.
Enquiry was notable as it evaluated the airline's corporate culture as contributing factor to the accident, namely management pressure leading to pilots flying in unsafe conditions. This was the first time an investigation took this approach to an air crash, which has become standard on ACI.
From the report (...) Although the Safety Board was unable to determine the reason(s) for the flight crew's deviation from SOP, it is believed that inordinate management pressures, the first officer's marginal instrument proficiency, the captain's inadequate supervision of the flight, inadequate crew training and procedures, and the captain's chronic fatigue were all factors in the accident (...)"
