∆ Mechanism- Organophosphates irreversibly inhibit acetylcholinesterase→ acetylcholine excess → cholinergic crisis (DUMBBELSS: Diarrhea, Urination, Miosis, Bradycardia, Bronchorrhea, Emesis, Lacrimation, Salivation, Seizures).
∆ Reason for this combination-:
Atropine: It is an Muscarinic antagonist→ dries secretions, increases Heart Rate, relieves bronchospasm (titrate to clear chest, dry skin).
Pralidoxime- It Reactivates AChE by cleaving phosphate bond before enzyme aging (give within 24-48hrs).
