Most dentists think the problem is their team, their systems, or their schedule.
But in 29 years, I’ve seen the truth:
Growth only happens when you pull the right lever.
Here are the 4 levers that top-performing dentists focus on:
Dental school offered one class on the business side of dentistry.
Most students skipped it.
They went to do one more root canal.
Then they graduated, hired a team, opened a practice, and discovered that clinical dentistry was only part of the job.
Leadership.
Communication.
Culture.
Hiring.
Financial decisions.
Nobody taught them how to be the CEO.
In this clip, Dr.Sonya Reddy explains why that gap still exists today and why so many dentists struggle after graduation despite being excellent clinicians.

The dentists who struggle most aren’t struggling clinically.
They’re struggling with everything dental school never taught.

The practice becomes valuable when the owner becomes optional.
I used to say obsolete. Same idea.
After 60+ practice transitions, I can tell you exactly what buyers are thinking when they walk into a practice where everything runs through the owner.
They’re thinking about risk.
What happens if this person gets sick?
What happens if they move to Hawaii?
What happens if they get hit by a bus?
In most practices, the answer is the same.
Everything stops.
And a practice that stops without its owner isn’t worth what the owner thinks it is.
I spent years making myself optional. Decentralized leadership. Clear accountability. Real ownership of responsibilities. I got to the point where I knew I could disappear for six months and the practice would keep running, keep producing, and keep taking great care of patients without a single call to me.
That’s not a management strategy.
That’s what makes a practice desirable instead of merely sellable.
Most dentists spend years trying to become indispensable.
The better goal is becoming optional.
Start now.
Not when you’re ready to sell.
Now.

Most dentists wait too long to think about their exit.
Not too long to sell.
Too long to think.
Most dentists spend decades building a practice and almost no time thinking about what they’re building it toward.
That’s the distinction Dr. Jeff Buske makes in this conversation.
A traditional broker shows up when you’re ready to leave.
A transition architect helps you build toward an outcome you actually chose.
If you don’t have a transition strategy, the market eventually picks one for you.
The goal isn’t to prepare to leave.
The goal is to build something strong enough that leaving becomes an option.

Most dentists think leaving academics is the scary part.
Dr. Arun Garg, DMD Arun Garg found out the real danger can come after the success.
He went from earning about $50,000 a year at the University of Miami to making in one week what used to take him an entire year.
Then the practice crashed.
Not because of his clinical skill.
Because of everything dental school never taught him.
How to market.
How to hire.
How to lead a team when nobody hands you a blueprint.
He was hiring anyone with a pulse. His words. And he’ll tell you it was almost always a mistake.
What saved him wasn’t becoming a better dentist.
It was admitting he needed a completely different kind of education and going to get it.
Most dentists who hit this wall never stop to ask why.
They blame the staff.
They blame the economy.
They blame the location.
They keep doing the same thing and wonder why nothing changes.
Clinical skill matters.
But skill alone won’t build the practice you envision.

The patient didn’t reject treatment.
The conversation simply stopped.
Most dentists assume case acceptance happens in the chair.
Many patients don’t make the final decision until days or weeks later.
The practices that win aren’t the ones diagnosing more treatment.
They’re the ones that stay in the conversation long enough for patients to act on treatment they already need.

One of the things I learned as a practicing dentist is that a lot of patients who want treatment never move forward.
Sometimes it’s fear.
Sometimes it’s timing.
A lot of times it’s money.
What I didn’t fully appreciate back then is how many patients simply disappear after a financing denial.
The assumption is that if one lender says no, the conversation is over.
But is it?
That was one of the topics Isaac Freckleton and I discussed on the Foundation Dental Podcast this week.
Isaac Freckleton is the Founder and CEO of Eve Financial. What interested me wasn’t another financing company. Dentistry already has plenty of those.
What interested me was the idea that many practices are making decisions based on a single lender’s opinion.
One lender may decline a patient.
Another may approve them.
One lender may approve a few hundred dollars.
Another may approve tens of thousands.
That’s a very different conversation.
The reality is that treatment acceptance isn’t just about clinical presentation. It isn’t just about communication. It isn’t just about trust.
Sometimes it’s about whether the patient has a realistic path to afford the care they already want.
As dentists, we spend a lot of time discussing production, collections, case acceptance, and growth.
We spend far less time discussing the financing systems sitting between treatment presentation and completed treatment.
Maybe we should.

The session ended at noon.
The conversations didn’t.
After our boardroom session that morning, the group spent the afternoon exploring Dubai, sharing experiences, and seeing the city together. That evening, we gathered at Armani Ristorante inside the Burj Khalifa.
No presentation.
No microphone.
No agenda.
Just a small group of dentists talking about business, leadership, family, health, and what the next chapter of life might look like.
I’ve spent more than 25 years in dentistry, and some of the most valuable insights I’ve gained never came from a lecture hall.
They came from conversations.
That’s what Luxury Dental Retreats are designed to create.
Not just education.
An environment where meaningful relationships and meaningful conversations can continue long after the session ends.
We’re bringing that experience to Santorini, Greece.
October 27 - November 1, 2026.
10 dentists.
18 CE hours.
Application required.

I spent hundreds of hours building the Dubai Luxury Dental Retreat because I refused to compromise on one thing.
Every person who walked out had to feel like they received more value than what it cost to attend.
That was the vision from day one.
On the Foundation Dental Podcast, Dr. Dua Farhoud, DDS shared something that was music to my ears.
She told me she felt like she may have gained more from the retreat than anyone else in the room.
Not because she had the most experience.
Because she had the least.
At the time, she was thinking about opening a practice. She was a blank slate.
She could have spent years making the same mistakes many of us made.
Instead, she spent a few days in a room full of people who had already lived them.
That’s the power of the right room.
The right room doesn’t just teach you something.
It saves you years.

Most dental conferences give you information. Very few give you the room where meaningful conversations actually happen.
This video was captured during a morning session at our 2025 Dubai Luxury Dental Retreat.
A small group of dentists gathered in a private boardroom overlooking the Arabian Gulf, talking about the things that matter most: leadership, practice growth, relationships, health, wealth, and how to build a life that actually works alongside a successful practice.
On this particular day, much of the discussion was led by Dr. Steven Rasner.
The format was conversational. No stage. No ballroom. No rushing between lecture halls. Just honest conversations among people facing many of the same opportunities and challenges.
What makes these retreats different isn’t just the faculty. It’s what happens after the session ends.
That afternoon we explored the Museum of the Future. Later we watched the sunset from the Burj Khalifa, shared champagne overlooking the Dubai skyline, and finished the evening with dinner at Armani Ristorante.
The conversations continued throughout the day and well into the evening.
That’s often where the most valuable insights emerge.
Not from a slide deck.
Not from a podium.
But from sitting across the table from someone who understands the journey because they’ve lived it themselves.
The education begins in the boardroom.
The relationships deepen everywhere else.
That’s the experience we’re bringing to Santorini, Greece, October 27 through November 1, 2026.
Your faculty:
Dr. Arun Garg, DMD - practice profitability, entrepreneurship, investing, and building wealth beyond dentistry.
Dr. Dr. Jeff Buske - leadership, health, relationships, and creating a life that works alongside a successful practice.
Elijah Desmond - EBITDA growth, practice value, and building a business worth more tomorrow than it is today.
Dr. Jim Arnold, DDS - fee-for-service transformation, PPO independence, and creating greater freedom, profitability, and control.
A boardroom, not a lecture hall.
Meaningful conversations.
Extraordinary experiences.

What you’re watching was captured at the 2025 Luxury Dental Retreat in Dubai at the Ritz-Carlton, JBR.
Conversational certified CE in a boardroom, not a lecture hall. No stage or sales pitches. A small group of dentists having the kinds of conversations that rarely happen at traditional conferences.
Private fine-dining. Curated experiences. Meaningful relationships. And a chance to step away from the day-to-day demands of practice ownership and think differently.
We’re bringing that experience to Santorini, Greece, October 27-November 1, 2026.
Your faculty:
Dr. Arun K Garg – Practice profitability, wealth creation, entrepreneurship, and using dentistry as a springboard to investments and opportunities beyond the practice.
Dr. Jeff Buske – Leadership, health, relationships, and creating a life that works alongside a successful practice.
Elijah Desmond – Increasing EBITDA, improving practice value, and positioning your business for a stronger future exit.

AI isn’t replacing dentists.
The practices that win over the next decade won’t be the ones that simply adopt AI. They’ll be the ones that combine it with something no algorithm produces.
Susan Leckowicz, Founder & CEO of Dental Coaches, said it directly during our conversation on the Foundation Dental Podcast.
AI can’t look a frightened patient in the eye and say, “You’re in really good hands. Dr. Arnold has done this procedure thousands of times.”
It can’t future pace someone back to the life they want.
“What’s it going to feel like to smile in pictures with your grandchildren again? You told me you missed that.”
It can’t place a hand on a shoulder when someone hasn’t been touched in months. That’s electric. And no system creates it.
AI can help polish your communication. It can help your team prepare for difficult conversations. It can make your systems faster and more efficient.
But the eye-to-eye connection that makes a patient feel safe, understood, and ready to say yes to treatment?
That’s yours.
It always will be.

When I graduated from dental school, I had $120,000 in debt.
Thirty years ago, that felt overwhelming.
I remember wondering how I was ever going to pay it back.
What I’ve learned since then is that debt usually isn’t the biggest problem.
Isolation is.
Most young dentists don’t fail because they lack intelligence or work ethic. They struggle because they’re trying to navigate a complicated profession without enough guidance from people who’ve already been there.
That’s why I’m such a believer in mentorship.
Not because mentors have all the answers.
Because they help you avoid years of unnecessary mistakes.
A recent conversation on Dr. Shahin Safarian’s live show reminded me of something I’ve believed for a long time:
You don’t have to reinvent the wheel.
Duplicate genius.
Find people who have already built the practice, career, relationships, and life you’re trying to create.
Learn from their successes.
Learn from their failures.
Condense time.
That’s how careers accelerate.
That’s how confidence grows.
And that’s how a mountain of debt eventually becomes a distant memory.

I recorded this video at the end of 2025.
At the time, a lot of what I talked about was still just an idea.
The Foundation Dental Mastermind was still being built. Future Luxury Dental Retreats were still being planned. Foundation Dental Transitions was gaining momentum. Many of the relationships, partnerships, and opportunities I was investing in hadn’t fully materialized yet.
Now we’re halfway through 2026, and it’s been one of the most rewarding years of my professional life.
I’ve attended 15 events so far this year. I’ve strengthened friendships and professional relationships that mean a great deal to me. The Mastermind continues to evolve. Santorini, Greece is on the calendar for our next Luxury Dental Retreat. And we’re actively helping several dentists navigate important practice transitions.
It’s a good reminder that sometimes the things you’re building take longer than you’d like.
You spend months, sometimes years, putting pieces into place without seeing immediate results. Then one day you look up and realize many of those efforts are finally starting to come together.
I’m incredibly grateful for the people who’ve been part of this journey, and I’m excited about what the second half of 2026 will bring.
The work continues.

At 50, Brett Kessler @bikodds wrote down what he wanted the second half of his life to look like.
That year:
He sold his practice.
He did his first Ironman.
He celebrated 20 years of sobriety.
He ran for the ADA Board of Trustees.
Eight years later he finished his term as the 161st President of the American Dental Association.
None of it was luck.
Every decision ran through one filter: am I moving closer to that vision, or further away?
That’s the whole system. Write the vision. Use it as a lens. Say yes to what moves you toward it. Say no to everything else.

A practice can be getting eight, nine, or ten times return on ad spend and still think the marketing isn’t working.
That’s one of the clearest points Gerritt Cora makes in this conversation.
Gerritt is the VP and Managing Partner of Progressive Dental Marketing, and in this clip he breaks down one of the biggest misunderstandings in dentistry:
A lot of owners are not evaluating marketing with financial clarity. They’re evaluating it emotionally.
That’s where the mistake happens.
In this clip, Gerritt explains why breakeven is often misunderstood, why most dentists compare marketing to the wrong benchmark, and why some campaigns are generating rates of return that would look extraordinary in almost any other category.
This isn’t just a marketing clip.
It’s a business clip.
It’s a leadership clip.
And for a lot of practice owners, it’s a financial literacy clip too.

This is one of the biggest mistakes dentists make.
They try to elevate their practice without elevating their environment.
On the Foundation Dental Podcast, Dr. Dua Farhoud, DDS shared what stood out most about attending the Dubai Luxury Dental Retreat.
It wasn’t the destination.
It wasn’t the CE.
It was the room.
A room full of dentists who weren’t making excuses, blaming circumstances, or settling for what was comfortable.
A room full of people focused on possibility.
That’s the part most people miss.
Your systems matter.
Your marketing matters.
Your team matters.
But the room you’re in often determines the ceiling you hit.
If you’re always the smartest person in the room, you’ve probably stayed there too long.
The right environment doesn’t just change what you know.
It changes what you believe is possible.

Most dentists don’t know they made the mistake until after the deal is done.
Eric J. Morin, MBA calls it the $10 million mistake.
Not because every dentist loses exactly $10 million.
Because too many owners confuse market value with economic value before they ever sit down at the table.
And too often, the people in that room - the broker, the financial advisor, the CPA, the bank - are incentivized to close fast.
Not necessarily to maximize what you walk away with.
In this clip from the Foundation Dental Podcast, Eric explains why dentists need to understand the true economic value of what they’ve built before they think about selling.
I share why this is so personal.
When I sold my group of practices, I learned how much leverage sophisticated buyers have when the seller doesn’t fully understand the game.
That’s why Foundation Dental Transitions exists.
Dentists shouldn’t walk into a gunfight with a feather.
They should know what maximizes value long before the transition conversation begins.
