Step 3: My Diagnostic Workup - "Less is More"
I don’t do shotgun testing. I do targeted testing:
For ALL: CBC, CRP, TSH, Tissue Transglutaminase IgA + Total IgA
If IBS-D or weight loss: Fecal Calprotectin, Stool studies for infection
If >50Y or red flags: Colonoscopy
I avoid: Random USG, CT, SIBO breath tests, food allergy panels in everyone
Goal: Rule out IBD, Celiac, Thyroid, Infection. That’s it.
Step 4: Classify the Subtype - Bristol Stool Form Scale
Treatment depends on this:
- IBS-C: >25% Bristol 1-2, <25% Bristol 6-7
- IBS-D: >25% Bristol 6-7, <25% Bristol 1-2
- IBS-M: >25% hard AND >25% loose
- IBS-U: Meets criteria but doesn’t fit
I ask patients to keep a 2-week stool diary. It changes our plan.

@adejoju_Ayofe So a stroke patient will present to physiotherapist before referral to doctor? You aren't serious or you don't even understand your job as a physiotherapist.
@kiloxtopher@Oladayo_@enodamade It's for the patient benefit. Doctor will discontinue a medication but nurses are still giving. You don't join rounds, don't read updated treatment plan. Just checking vitals.
