Optimal serum K+ concentrations in heart failure: an individual patient data meta-analysis

📝Pooled patient-level analysis of 12 #RCTs (32,346 #HFrEF + 13,723 #HFpEF pts):

Serum K⁺ showed a reverse J-shaped relationship with outcomes in #HFrEF.

Compared with K⁺ 4.0–4.5 mmol/L:
📍K⁺ <3.5 mmol/L → ↑ all-cause mortality (aHR 1.49, 95% CI 1.27–1.76) , ↑ CV death, sudden death & pump failure death

Lowest risk observed at K⁺ 4.2–5.0 mmol/L.
Notably, “mild hyperkalaemia” (5.0–5.5 mmol/L) was NOT associated with worse outcomes
